Enrolling: 

• Please complete the online application under Parent & Families Section select Student Enrollment Application

• If you have a printed copy- please return to us by fax (336-597-2124) , email (andersl@person.k12.nc.us) or to the school

• Send a copy of your child's birth certificate (or we can make a copy)

• Child's full name & birth date

• Does your child have Medicaid or Insurance?

• Parent/Guardian's name, address, telephone number and email address

• Services the child currently receives (if any) - Child Care, Medical and/ or Developmental
